Waiver of Liability

By checking the boxes and digitally signing below, I acknowledge that I have read, understand, and fully agree to the following disclaimer.

I acknowledge that Griffin Coombs and his associates are natural health practitioners and do not diagnose, cure, or treat any illness or disease. Further, the undersigned fully releases The North Star Body, Griffin Coombs, his lab partners, his independent representatives, associates, and affiliates from any and all liability for any failure to identify any medical condition or disease. It is understood and agreed that this is not the purpose of their natural health services.

I understand that Griffin Coombs (referred to hereby as "instructor") is not a registered medical practitioner, nor is anyone else at Oxygen Research Institute Ltd. No advice and activity presented, demonstrated or advised during any sessions with the instructor are in any way intended as a substitute for a medical consultation, and should not replace or interfere with any guidance offered by a medical professional.

I understand that I am free to stop activity during any session at any time, and for any reason. If at any time during any session, I feel the need for any assistance, medical or otherwise, I agree to cease activity and notify the instructor immediately, and take full responsibility for responding to my needs, including stopping the session and obtaining appropriate care. If I fail to seek the required medical care or ignore medical advice, or safety advice from the instructor, I understand and agree to do so at my sole risk.

I understand I will need to inform my instructor about my pregnancy status, if any, before starting each session. If I become pregnant or believe I may be pregnant after starting sessions, I agree to stop all breathing exercises immediately and inform the instructor to guide me on the next course of action.

I hereby confirm that I have carefully read this disclaimer and have fully understood that this is a release of liability. I hereby expressly agree to release and discharge Griffin Coombs and The North Star Body, as well as anybody associated with Oxygen Research Institute Ltd. (including its employees, directors, and/or management) from any and all claims or causes of action and agree to waive any right that I may otherwise have to bring legal action against said individuals and corporations for personal injury and/or damage to property.

Electronic Signature Consent

By checking the boxes and signing below, I hereby agree to sign this document electronically by typing my legal name below. I affirm that my electronic signature, and all future electronic signatures, were signed by myself with full knowledge and consent, and that I am legally bound to these terms and conditions. I understand that I may opt out of electronically signing, and that no services may be rendered until either an electronic or paper copy of this acknowledgement has been signed.
